We collect the personal information you provide when you book an appointment, complete patient forms, or contact us — such as your name, contact details, insurance information, and health history relevant to your dental care.
How we use it
Your information is used to provide dental care, manage appointments and billing, process insurance claims with your consent, and communicate with you about your care. As a health information custodian in Ontario, we handle personal health information in accordance with the Personal Health Information Protection Act (PHIPA).
Third-party services
Online booking is provided through a third-party scheduling service (NexHealth); this website also uses embedded services (such as Google Maps) that may set their own cookies.
Your choices
You may request access to or correction of your personal information, and you may withdraw consent for uses that aren't required to provide your care. Contact our office and we'll help.
